Details of Care Management Program

• You may text or email any questions or concerns while participating in the program. I typically check for messages and try to respond twice a day but do not guarantee a response for 48 hours. There will be limited times when I am out of the office and unable to respond for more than 48 hours. You will be notified ahead of time during these times.

• This program’s success relies on a healthy, supportive relationship that is also respectful of my time. I understand that your pet’s health is very important to you, and I try to make myself available for your needs in a timely manner. However, if I feel that you are unable to keep healthy boundaries to allow for my well being then I have the right to discontinue my services at any time for any reason and a pro-rated refund will be given.
